After booking, you’ll receive an intake form to complete before your first appointment. Please set aside some time to complete it thoughtfully.

Please note that a credit card is required to be on file for booking.

• Your card will not be charged unless you cancel within the cancellation period (2 business days) or don’t show up to your appointment.

• The cancellation fee will be up to the full cost of the visit fee.

• By booking an appointment, you are agreeing to this policy.
